XRP Ledger developers have proposed four fixes to prevent restricted tokens from entering accounts without issuer approval. These fixes have been included in the fixCleanup3_5_0 network upgrade but have not yet been activated on the mainnet. The XRP Ledger Foundation said these changes are routine protocol hardening, not a response to a security incident. No reports of users being affected have been made so far.
