When everything seemed under control, the execution stopped feeling like a decision and started to feel like a procedure. The person no longer approached the review with the same willingness because the previous experience had been consistent. Nothing failed the first time. Nor the second. Even the third reinforced the idea that the process was mastered.
What changed was not the system, but the relationship with it. Attention shifted from active to residual. The conditions were not reviewed with the same rigor because there was no immediate reason to do so. Each successful repetition reduced the perception of risk, not because the risk had disappeared, but because it was no longer being measured.
The error did not occur in an isolated action. It consolidated in the repetition. At the moment something deviated slightly, there was no clear alert because the person was no longer looking for signals. The margin for correction still existed, but it went unnoticed. When the impact became visible, the decision had already been executed several times under a false sense of stability.
I thought I had it resolved… and that was when I stopped reviewing.
